The certificate of analysis format used by the independent laboratory shall conform to the format specified by IJEC and shall include, at a minimum, the following information.

• Supplier name
• Material designation (INCI name and trade name, if applicable)
• Lot number
• IJEC specified quality requirements (chemical/physical property, minimum/maximum, range)
• Methods used
• Actual results
• Date tested
• Name of sampling service
